To update a minor’s information, complete a Change of Data for Minors Form, obtainable from the Customer Care Office. The form must be signed by both parents.
Please submit the following documents either in person at the MDH Customer Care Office or via email to [email protected]:

• Both parents’ ID cards
• The child’s birth certificate
• Custody documentation (if applicable)

You may update your personal information by completing and signing a Change of Data Form, available from the Customer Care Office or by sending your request for this form via the email provided hereunder. A valid ID card must be presented when submitting the form in person, or you may email the signed form along with a copy of your ID to: [email protected].
