Yes, results can be sent securely via email once the Personal Medical request for data form and your consent are provided. (One can also request this form through the generic email)

You may request copies in the following ways:

• In person: Visit the Customer Care Office with your valid ID. Obtain the Personal Medical Request for Data Form there. (One can also request this form through the generic email listed below.)
• By email: Complete and sign the Personal Medical Request for Data Form, attach a scanned copy of your ID, and email it to [email protected].

There are no fees for the first request. Fees apply for any additional or repeated requests.
